You must complete the form and send it to us as soon as possible. If you don't, the date you are paid benefit from could be delayed. If you let us know that you have downloaded a form, either by phone (01452 396483) or e mail (benefits@gloucester.gov.uk ) and subsequently return the form to us within 1 calendar month, we will pay any weekly benefit due to you from the Monday following the date you actually downloaded the form.
