Downloading Forms

You can download claim forms in PDF format and print them out to fill in with a pen.

Please note forms are in colour. We advise you to print them in 'greyscale'. See print properties or options on your printer.

You must complete the form and send it to us as soon as possible. If you don't, the date you are paid benefit from could be delayed. If you let us know that you have downloaded a form, either by phone (01452 396483) or e mail (benefits@gloucester.gov.uk ) and subsequently return the form to us within 1 calendar month, we will pay any weekly benefit due to you from the Monday following the date you actually downloaded the form.

• An electronic Housing Benefit and/or Council Tax Rebate form.   The quickest and easiest way to apply for Housing Benefit and/or Council Tax Rebate is to apply on-line using our eform.  At the end of the form submit your information and you will be asked to provide evidence to support your claim.
• Change Of Circumstances Form - Complete this form if you, or anyone else on your claim, have had any changes in your circumstances.
• Discretionary Housing Payment Form - complete this form if you require additional assistance to meet your rent and/or council tax (We will   consider whether additional help with housing costs and council tax is needed.)
• Earnings form - the form for your employer to complete, as evidence, if you do not yet have the number of payslips we require
• Hostel Form - Fill in this form if you are a hostel resident
• Housing Benefit and Council Tax Rebate (new claims) - Complete this form if you wish to apply for housing benefit and council tax rebate (notes to help you fill in this form)
• Landlords - If you want to have your benefit paid directly to your landlord please click here to download a form for completion
• Pensioners - Children Form - Complete this form if you have a child/children for whom you are responsible
• Pensioners - Housing and Council Tax Form - If you are a pensioner download this special short form to apply for housing benefit and council tax rebate
• Pensioners - Other People Who Live With You - Complete this form when you have other people living with you (not including your partner)
• Pensioners - Rent Details - If you rent your property from a private landlord you will also need to complete this form
• Rent schedule - Form for your landlord to complete, as evidence of payments, if you do not have a rent book
• Safeguard Application From - If your Housing Benefit is calculated using Local Housing Allowance, we will normally make payments direct to you. If, however, you feel you are unable to manage you finances due to one of the reasons mentioned in our Safeguard Policy, please use this form to tell us about your situation.
• Second Adult Rebate Form - complete this form if you think you are entitled to a Second Adult Rebate
• Self Employed -Notes To Help You Complete the Self Employed Form
• Self Employed -  Additional Information Form - Complete this form if you are self employed
• Students - If you are a student fill in this form along with the main housing benefit and council tax rebate form

If you have a change in the circumstances of anyone on your claim please use one of these forms to tell us:

• Change of address form - Complete this form if you have moved - tick the front of the form to let us know you are reporting a change of your address (notes to help you fill in this form)
• End of Income support/Jobseeker's allowance/Employment and Support allowance benefit form. - Complete this form if you have stopped receiving income support,  Jobseeker's allowance or employment and support allowance - tick the front of the form to let us know (notes to help you fill in this form)
